RESUMEN

The fifth-generation (5G) cellular communication technology introduces technical advances that can expand medical device access to connectivity services. However, assessing the safety and effectiveness of emerging 5G-enabled medical devices is challenging as relevant evaluation methods have not yet been established. In this paper, we propose a design model for 5G testbed as a regulatory science tool (TRUST) for assessing 5G connectivity enablers of medical device functions. Specifically, we first identify application specific testing needs and general testing protocols. Next, we outline the selection and customization of key system components to create a 5G testbed. A TRUST demonstration is documented through a realistic 5G testbed implementation along with the deployment of a custom-built example use-case for 5G-enabled medical extended reality (MXR). Detailed configurations, example collected data, and implementation challenges are presented. The openness of the TRUST design model allows a TRUST testbed to be easily extended and customized to incorporate available resources and address the evaluation needs of different stakeholders.

Introduction To establish a centralized inventory management system for the efficient functioning of all healthcare facilities, e-Upkaran (equipment management and maintenance system) was launched in 2015 in the state of Rajasthan. This study is conducted to assess the functioning of e-Upkaran in Rajasthan. Methods The assessment of the e-Upkaran system for primary and secondary healthcare centers was carried out using a systematic review of the literature and a multi-indicator stakeholder questionnaire. The benefits evaluation framework focused on the system quality, information and service quality, use and user satisfaction, and net benefits utilized for the assessment. A review of the literature was done to highlight the importance of computerized medical equipment management and maintenance systems and appraise the challenges and benefits associated with such systems as compared to the traditional pen-paper register. Information was gathered based on available documents, field observation, and data obtained from specific hospital staff, including the bioengineers and other users of e-Upkaran. Results The finding of this study suggests that e-Upkaran efficiently improves documentation, reporting, maintenance, and management of medical equipment. It is more efficient than the traditional paper-pen system. It is designed to minimize downtime and maintain equipment in good operating condition and has potential benefits in terms of improving information quality, use, and net benefit. The cost of service ratio is within the benchmark value. This system has also considerably reduced out-of-pocket expenditure. Computer proficiency and the workload of other e-health programs pose a challenge in the implementation of this program. Conclusion The e-Upkaran system is competent in terms of improving information quality, use, and net benefit. Other Indian states could also adopt this system to improve their biomedical equipment management and maintenance system.

BACKGROUND: Contactless vital signs (VS) measurement with video photoplethysmography (vPPG), motion analysis (MA), and passive infrared thermometry (pIR) has shown promise. OBJECTIVES: To compare conventional (contact-based) and experimental contactless VS measurement approaches for emergency department (ED) walk-in triage in pandemic conditions. METHODS: Patients' heart rates (HR), respiratory rates (RR), and temperatures were measured with cardiorespiratory monitor and vPPG, manual count and MA, and contact thermometers and pIR, respectively. RESULTS: There were 475 walk-in ED patients studied (95% of eligible). Subjects were 35.2 ± 20.8 years old (range 4 days‒95 years); 52% female, 0.2% transgender; had Fitzpatrick skin type of 2.3 ± 1.4 (range 1‒6), Emergency Severity Index of 3.0 ± 0.6 (range 2‒5), and contact temperature of 36.83°C (range 35.89-39.4°C) (98.3°F [96.6‒103°F]). Pediatric HR and RR data were excluded from analysis due to research challenges associated with pandemic workflow. For a 30-s, unprimed "Triage" window in 377 adult patients, vPPG-MA acquired 377 (100%) HR measurements featuring a mean difference with cardiorespiratory monitor HR of 5.9 ± 12.8 beats/min (R = 0.6833) and 252 (66.8%) RR measurements featuring a mean difference with manual RR of -0.4 ± 2.6 beats/min (R = 0.8128). Subjects' Emergency Severity Index components based on conventional VS and contactless VS matched for 83.8% (HR) and 89.3% (RR). Filtering out vPPG-MA measurements with low algorithmic confidence reduced VS acquired while improving correlation with conventional measurements. The mean difference between contact and pIR temperatures was 0.83 ± 0.67°C (range -1.16-3.5°C) (1.5 ± 1.2°F [range -2.1-6.3°F]); pIR fever detection improved with post hoc adjustment for mean bias. CONCLUSION: Contactless VS acquisition demonstrated good agreement with contact methods during adult walk-in ED patient triage in pandemic conditions; clinical applications will need further study.

The advancement of technology in medical equipment has significantly improved healthcare services. However, failures in upkeeping reliability, availability, and safety affect the healthcare services quality and significant impact can be observed in operations' expenses. The effective and comprehensive medical equipment assessment and monitoring throughout the maintenance phase of the asset life cycle can enhance the equipment reliability, availability, and safety. The study aims to develop the prioritisation assessment and predictive systems that measure the priority of medical equipment's preventive maintenance, corrective maintenance, and replacement programmes. The proposed predictive model is constructed by analysing features of 13,352 medical equipment used in public healthcare clinics in Malaysia. The proposed system comprises three stages: prioritisation analysis, model training, and predictive model development. In this study, we proposed 16 combinations of novel features to be used for prioritisation assessment and prediction of preventive maintenance, corrective maintenance, and replacement programme. The modified k-Means algorithm is proposed during the prioritisation analysis to automatically distinguish raw data into three main clusters of prioritisation assessment. Subsequently, these clusters are fed into and tested with six machine learning algorithms for the predictive prioritisation system. The best predictive models for medical equipment's preventive maintenance, corrective maintenance, and replacement programmes are selected among the tested machine learning algorithms. Findings indicate that the Support Vector Machine performs the best in preventive maintenance and replacement programme prioritisation predictive systems with the highest accuracy of 99.42 and 99.80%, respectively. Meanwhile, K-Nearest Neighbour yielded the highest accuracy in corrective maintenance prioritisation predictive systems with 98.93%. Based on the promising results, clinical engineers and healthcare providers can widely adopt the proposed prioritisation assessment and predictive systems in managing expenses, reporting, scheduling, materials, and workforce.

Medical equipment highly contributes to the effectiveness of healthcare services quality. Generally, healthcare institutions experience malfunctioning and unavailability of medical equipment that affects the healthcare services delivery to the public. The problems are frequently due to a deficiency in managing and maintaining the medical equipment condition by the responsible party. The assessment of the medical equipment condition is an important activity during the maintenance and management of the equipment life cycle to increase availability, performance, and safety. The study aimed to perform a systematic review in extracting and categorising the input parameters applied in assessing the medical equipment condition. A systematic searching was undertaken in several databases, including Web of Science, Scopus, PubMed, Science Direct, IEEE Xplore, Emerald, Springer, Medline, and Dimensions, from 2000 to 2020. The searching processes were conducted in January 2020. A total of 16 articles were included in this study by adopting Preferred Reporting Items for Systematic Review and Meta-Analyses (PRISMA). The review managed to classify eight categories of medical equipment reliability attributes, namely equipment features, function, maintenance requirement, performance, risk and safety, availability and readiness, utilisation, and cost. Applying the eight attributes extracted from computerised asset maintenance management system will assist the clinical engineers in assessing the reliability of medical equipment utilised in healthcare institution. The reliability assessment done in these eight attributes will aid clinical engineers in executing a strategic maintenance action, which can increase the equipment's availability, upkeep the performance, optimise the resources, and eventually contributes in providing effective healthcare service to the community. Finally, the recommendations for future works are presented at the end of this study.

Assessment of autonomic nervous system (ANS) functioning may be performed non-invasively using autonomic tests which are based on evaluation of response of cardiovascular system to the applied stimuli, such as increased air pressure during Valsalva maneuver, skeletal muscle contraction during static handgrip or deep slow breathing. The cardiovascular response depends, besides ANS reaction and test protocol, also on the way stimulus is self-applied by the test subject. We present a versatile device for controlling stimulus self-application during three ANS tests: Valsalva maneuver, static handgrip, and deep breathing. It integrates two different gauges and a pace setter for breathing into one device. The core of the device is a linear LED display which, using green, yellow, and red diodes, informs the subject about the correctness of self-application of respective stimulus. The settings of the device can be adjusted to the needs of the protocol chosen. The device can record the duration of mouth air pressure or the force produced by the subject during ANS tests, which assures correctness of the tests, thus allowing to track individual variability changes in the response to the test. The device was verified during ANS tests and its use was intuitive for patients, reducing the time needed for training before tests and decreasing the effort of the physician.

La presente propuesta de diseño busca la interoperabilidad de datos en salud por medio de la integración de un sistema de comunicación a equipos especializados, utilizando las TI (Tecnologías de la Información) y el IoT (Internet de las Cosas) en los equipos biomédicos especializados o monitor que está siendo utilizado actualmente en los centros hospitalarios público a nivel nacional para la medición de signos vitales, con el objetivo de poder brindar una atención sanitaria a pacientes con padecimientos médicos y que necesitan de una constante supervisión y vigilancia. En los centros hospitalarios se cuenta con equipo biomédico especializado que carece de dichas funcionalidades. Esto repercute u obliga al responsable especialista a tener que estar frente al monitor y paciente para poder observar las mediciones o lecturas que presenta un paciente.


This design proposal seeks the interoperability of health data through the integration of a communication system to specialized equipment, using IT (Information Technology) and IoT (Internet of Things) in specialized biomedical equipment or monitor that is currently being used in public hospitals nationwide for the measurement of vital signs, in order to provide health care to patients with medical conditions and who need constant surveillance. Hospitals have specialized biomedical equipment that lacks these functionalities. This forces the responsible specialist to have to be in front of the monitor and the patient to be able to observe the measurements or readings that a patient presents.

The field of Open Source Hardware Mechanical Ventilators (OSH-MVs) has seen a steep rise of contributions during the 2020 COVID-19 pandemic. As predictions showed that the number of patients would exceed current supply of hospital-grade ventilators, a number of formal (academia, the industry and governments) and informal (fablabs and startups) entities raced to develop cheap, easy-to-fabricate mechanical ventilators. The presence of actors with very diverse modus operandi as well as the speed at which the field has grown, led to a fragmented design space characterized by a lack of clear design patterns, projects not meeting the minimum functional requirements or showing little-to-no innovation; but also valid alternatives to hospital-grade devices. In this paper we provide a taxonomic system to help researchers with no background in biomedical engineering to read, understand and contribute to the OSH-MV field. The taxonomy is composed of ten properties that are read through the lenses of three reflection criteria: buildability, adoptability and scalability. We applied the taxonomy to the analysis of seventeen OSH-MV projects, which are representative of the current landscape of possibilities available for COVID-19 patients. We discuss the different design choices adopted by each project highlighting strengths and weaknesses and we suggest possible directions for the development of the OSH-MV field.

Ballistocardiogram (BCG) is a graphical representation of the subtle oscillations in body movements caused by cardiovascular activity. Although BCGs cause less burden to the user, electrocardiograms (ECGs) are still commonly used in the clinical scene due to BCG sensors' noise sensitivity. In this paper, a robust method for sleep time BCG measurement and a mathematical model for predicting sleep stages using BCG are described. The novel BCG measurement algorithm can be described in three steps: preprocessing, creation of heartbeat signal template, and template matching for heart rate variability detection. The effectiveness of this algorithm was validated with 99 datasets from 36 subjects, with photoplethysmography (PPG) to compute ground truth heart rate variability (HRV). On average, 86.9% of the inter-beat intervals were detected and the mean error was 8.5ms. This shows that our method successfully extracted beat-to-beat intervals from BCG during sleep, making its usability comparable to those of clinical ECGs. Consequently, compared to other conventional BCG systems, even more accurate sleep heart rate monitoring with a smaller burden to the patient is available. Moreover, the accuracy of the sleep stages mathematical model, validated with 100 datasets from 25 subjects, is 80%, which is higher than conventional five-stage sleep classification algorithms (max: 69%). Although, in this paper, we applied the mathematical model to heart rate interval features from BCG, theoretically, this sleep stage prediction algorithm can also be applied to ECG-extracted heart rate intervals.

Economic and technological development advances exponentially, and the implementation of new technologies in the health sector has become a source of waste for electrical and electronic equipment (WEEE). Electrical and electronic equipment must be replaced periodically, either due to a technological update or to improve medical treatments, which ultimately leads to the generation of this type of waste. This work aimed to conduct exploratory research on the current situation of the handling of this type of waste in Barranquilla, Colombia, considering the limited information related to the management of biomedical WEEE in the city. Sixty health care institutions (HCIs), including hospitals and clinics, odontological centres and ophthalmological centres, participated favourably in the surveys concerning the management of WEEE. Through this work, it was possible to establish a working staff level of knowledge on WEEE disposal and the framework for the management and temporary collection of this waste. Therefore, the data are useful for proposing strategies for the integral management of electrical and electronic waste in both small and large populations.

Implantable motor neuroprosthetic systems can restore function to individuals with significant disabilities, such as spinal cord injury, stroke, cerebral palsy, and multiple sclerosis. Neuroprostheses provide restored functionality by electrically activating paralysed muscles in coordinated patterns that replicate (enable) controlled movement that was lost through injury or disease. It is important to consider the general topology of the implanted system itself. The authors demonstrate that the wired multipoint implant technology is practical and feasible as a basis for the development of implanted multi-function neuroprosthetic systems. The advantages of a centralised power supply are significant. Heating due to recharge can be mitigated by using an actively cooled external recharge coil. Using this approach, the time required to perform a full recharge was significantly reduced. This approach has been demonstrated as a practical option for regular clinical use of implanted neuroprostheses.

A soft and highly directive, proximity-coupled split-ring resonator fabricated with a liquid alloy, copper and polydimethylsiloxane (PDMS) is presented. The same was designed for sensing osteogenesis of calvarial bone. As dielectric properties of bone grafts in ossifying calvarial defects should change during the osteogenesis process, devices like this could monitor the gradual transformation of the defect into bone by differentiating changes in the dielectric properties as shifts in the resonance frequency. Computational Software Technology (CST) Microwave Studio®-based simulation results on computational head models were in good agreement with laboratory results on head phantom models, which also included the comparison with an in-vivo measurement on the human head. A discussion based on an inductive reasoning regarding dynamics' considerations is provided as well. Since the skin elasticity of newborn children is high, stretching and crumpling could be significant. In addition, due to typical head curvatures in newborn children, bending should not be a significant issue, and can provide higher energy focus in the defect area and improve conformability. The present concept could support the development of soft, cheap and portable follow-up monitoring systems to use in outpatient hospital and home care settings for post-operative monitoring of bone healing after reconstructive surgical procedures.

Accidental falls of patients cannot be completely prevented. However, timely fall detection can help prevent further complications such as blood loss and unconsciousness. In this study, the authors present a cost-effective integrated system designed to remotely detect patient falls in hospitals in addition to classifying non-fall motions into activities of daily living. The proposed system is a wearable device that consists of a camera, gyroscope, and accelerometer that is interfaced with a credit card-sized single board microcomputer. The information received from the camera is used in a visual-based classifier and the sensor data is analysed using the k-Nearest Neighbour and Naïve Bayes' classifiers. Once a fall is detected, an attendant at the hospital is informed. Experimental results showed that the accuracy of the device in classifying fall versus non-fall activity is 95%. Other requirements and specifications are discussed in greater detail.

The performance of an acoustic stethoscope is improved by translating, without loss of fidelity, heart sounds, chest sounds, and intestinal sounds below 50 Hz into a frequency range of 200 Hz, which is easily detectable by the human ear. Such a frequency translation will be of significant benefit to hearing impaired physicians and it will improve the stethoscope performance in a noisy environment. The technique is based on a single sideband suppressed carrier modulation. Stability and bias problems commonly associated with an analog frequency translator are avoided by an all-digital implementation. Real-time audio processing is made possible by approximating a Hilbert transformer with a time delay. The performance of the digital frequency translator was verified with a 16-bit 44.1 Ks/s audio coder/decoder and a 32-bit 72 MHz microcontroller.

Venous leg ulcerations are a common problem, with high prevalence in the middle-aged and elderly population, and more attention on research of their physical activities has been paid, as they have great effects on the blood circulation of the lower limb. With enough, appropriate training, the chronic venous ulcerations in the lower limb can be avoided and alleviated, and venous hypertension can be reduced effectively. The study deals with a physical activity tracking system for the patients based on a three-axis accelerometer. The system uses a three-axis accelerometer, a microcontroller, and a wireless Bluetooth module to form a data acquisition platform to acquire accelerations of the lower limb movement, and sends it to a smart mobile phone via the wireless Bluetooth module. The system takes advantages of the smart mobile phone to guide the chronic venous leg ulcers to do prescribed rehabilitation exercises for the lower limb muscles, perform acceleration data preprocessing, wavelet transform and reconstruction, denoising and feature extraction, obtain the results of the rehabilitation exercises, and then give reasonable evaluation and judgment. It is helpful to treat underlying venous reflux, create such an environment that allows skin to grow across an ulcer, and accelerate ulcer healing process consequently.

Resumen Se analizan 112 auditorías de instalaciones eléctricas y seguridad de equipos biomédicos en 78 Institutos de Medicina Altamente Especializada (IMAE) del Uruguay, realizadas a lo largo de 14 años, clasificando el nivel de riesgo y de cumplimiento de normas desde el punto de vista de Ingeniería Clínica. Cada visita incluye una encuesta al personal encargado de mantener y gestionar la infraestructura eléctrica y el equipamiento biomédico, que abarca el estado de mantenimiento, el control y la documentación de las instalaciones eléctricas y del equipamiento biomédico. Se evalúa el riesgo con un puntaje de 0 a 4. En 2004-2007 el 74% de los IMAE tenía irregularidades en la instalación eléctrica, gestión de equipamiento, control de calidad o documentación. Además, un 15% de los que tenían problemas, tenía en particular equipamiento indicado como "equipo peligroso". En los períodos siguientes esta proporción baja paulatinamente hasta 0% en 2016-2017. No obstante, continúa existiendo un déficit en la gestión del equipamiento y en la documentación formal. El aporte de la Universidad en el seguimiento técnico de los IMAE se ha materializado en una mejora en materia de seguridad.


Abstract 112 field inspections to 78 high technology medical centers (IMAE is the Spanish acronym) over 14 years are analyzed. All visits were evaluated as to Clinical Engineering good practices and were assigned a risk level. All audits included a questionaire to maintenance management personnel on electrical network operation as well as on biomedical equipment follow-up and documentation from acquisition to disposal. Risk is assigned a level 0 to 4 at each visit. In 2004-2007, 74% of IMAEs had safety problems in one or more of electrical network, maintenance management or documentation, and 15% of the IMAEs with safety problems had one piece of equipment described as simply "dangerous". Electrical safety problems were eventually reduced to 0% in 2016-2017, probably as a consequence of regular audit and counseling by this University Clinical Engineering Program.

RESUMO: Introdução: O Método Therasuit possui alto custo e não é disponibilizado pelo Sistema Único de Saúde, o que tem ocasionado a judicialização desse tratamento. O estudo visou traçar o perfil dessa judicialização, bem como estimar os custos diretos decorrentes do cumprimento das decisões judiciais deferidas. Método: Foram analisados processos submetidos ao Tribunal de Justiça do Rio de Janeiro entre janeiro de 2013 e janeiro de 2017, no qual foi solicitado o Método Therasuit. Os dados demográficos, clínicos, advocatícios e jurídicos foram extraídos, assim como o tempo da decisão judicial e os orçamentos da tecnologia requerida. Resultados: O total de 11 processos foi analisado. Os autores tinham uma média de idade de 6,8 anos e mediana de 6, sendo a maioria do sexo masculino e residentes na capital do estado. A quadriparesia foi a condição mais relatada. A gratuidade de justiça foi solicitada por todos, e a Defensoria Pública foi utilizada em 9 dos 11 processos analisados. Foram consideradas as decisões judiciais em primeira instância. O tempo da decisão judicial foi em média de 266,5 dias com uma mediana de 35,5. Em todos houve requerimento da ferramenta jurídica tutela antecipada. O índice de deferimento foi de 90%, totalizando um custo direto anual de R$501.894,09. Discussão: A judicialização desse tratamento pode ocasionar um deslocamento não previsto de verba pública, podendo impactar nos princípios da equidade e na integralidade do Sistema Único de Saúde. Conclusão: Constatou-se elevada taxa de processos deferidos, acarretando um alto custo despendido pelo Poder Público para atender a uma pequena parcela de pacientes.


ABSTRACT: Introduction: The lack of availability of the Therasuit Method by the Unified Health System associated with its high cost has led to the prosecution of this treatment. The study aimed to outline the profile of this judicialization, as well as to estimate the direct costs resulting from compliance with the deferred judicial decisions. Method: Weanalyzed the cases submitted to the Court of Justice of Rio de Janeiro between January 2013 and January 2017, in which the Therasuit Method was applied. Demographic, clinical, advocacy and legal data were extracted, as was the timing of the court's decision and the required technology budgets. Results: Atotal of 11 processes was analyzed. The authors had a mean age of 6.8 years and a median of 6, the majority being male, and resident in thestate capital. Quadriparesis was the most reported condition. The gratuity of justice was requested by all, and the Public Defender's Officewas used by 9 of the 11 processes. The judicial decisions at first instance were considered. In all of processes there was application of the legal tool called guardianship. The time of the judicial decision was on average of 266.5 days with a median of 35.5. The deferral index was 90%, totaling an annual direct cost of R$501,894.09. Discussion: The judicialization of this treatment can cause an unforeseen displacement of public funds, transgressing the principles of equity and the integrality of Unified Health System. Conclusion: It was observed a high rate of deferred processes, resulting in a high cost spent by the Public Power to attend a small portion of patients.

RESUMEN

The progress of microelectromechanical systems tends to fabricate miniature motion sensors that can be used for various purposes of biomedical systems, particularly on-body applications. A miniature wireless sensor is developed that not only monitors heartbeat and respiration rate based on chest movements but also identifies initial problems in the cardiorespiratory system, presenting a healthy measure defined based on height and length of the normal distribution of respiration rate and heartbeat. The obtained results of various tests are compared with two commercial sensors consisting of electrocardiogram sensor as well as belt sensor of respiration rate as a reference (gold standard), showing that the root-mean-square errors obtain <2.27 beats/min for a heartbeat and 0.93 breaths/min for respiration rate. In addition, the standard deviation of the errors reaches <1.26 and 0.63 for heartbeat and respiration rates, separately. According to the outcome results, the sensor can be considered an appropriate candidate for in-home health monitoring, particularly early detection of cardiovascular system problems.

Diabetic patients are prone to daily and severe health-related risks, namely hyper and hypoglycaemia. Hypoglycaemia phenomenon happens when the glucose level in patient's blood is lower than a well-determined sill. It may induce serious impacts, such as functional brain failure or even the death. Hypoglycaemia is especially dangerous when it occurs during the night while the patient is asleep because it becomes difficult to be detected by the patient itself or other persons around him. While all existing sensor-based solutions are detection-only driven, the proposed solution goes beyond and attempts to treat autonomously, and at low cost, the nocturnal hypoglycaemia. The presented system detects the nocturnal hypoglycaemia phenomenon based on accelerated heart-rate symptom and a progressive detection algorithm. The system treats then the detected nocturnal hypoglycaemia throughout safe and automatic injection of glucagon.

Interventional C-arm imaging is crucial to percutaneous orthopedic procedures as it enables the surgeon to monitor the progress of surgery on the anatomy level. Minimally invasive interventions require repeated acquisition of X-ray images from different anatomical views to verify tool placement. Achieving and reproducing these views often comes at the cost of increased surgical time and radiation. We propose a marker-free 'technician-in-the-loop' Augmented Reality (AR) solution for C-arm repositioning. The X-ray technician operating the C-arm interventionally is equipped with a head-mounted display system capable of recording desired C-arm poses in 3D via an integrated infrared sensor. For C-arm repositioning to a target view, the recorded pose is restored as a virtual object and visualized in an AR environment, serving as a perceptual reference for the technician. Our proof-of-principle findings from a simulated trauma surgery indicate that the proposed system can decrease the 2.76 X-ray images required for re-aligning the scanner with an intra-operatively recorded C-arm view down to zero, suggesting substantial reductions of radiation dose. The proposed AR solution is a first step towards facilitating communication between the surgeon and the surgical staff, improving the quality of surgical image acquisition, and enabling context-aware guidance for surgery rooms of the future.
